• "Parent 2" is only needed if you would like to include another caregiver's information
• "Player Photo," "Birth Certificate," are useful when your child registers for team play (U6 and older), but not yet required
• "Signed Waiver," "Release of Liability" are NOT required, so please ignore -- no need to upload anything!
